Developmental Disability Support Coordinator
Duties: Responsible for providing ongoing DD Support Coordination to adults and children within the catchment area of Rockbridge Area Community Services. Services include, but are not limited to: monitoring services, referrals to other agencies/services, assessments and the completions of required documentation; knowledge of treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning and service coordination; abilities to work as a team member, maintain effective inter-and intra-agency working relationships, work independently performing position duties under general supervision, engage and sustain ongoing relationships with individuals receiving services; skills in identifying and documenting how resources, services and natural supports such as family can be utilized to promote achievement of an individual’s personal habilitative/rehabilitative and life goals.
Qualifications: Must possess a Bachelor’s Degree in the human services field or hold a license as a RN in VA and have a minimum of one year of documented experience working directly with individuals who have intellectual disability or other developmental disabilities in developing, conducting and approving assessments, ISPs or treatment plans meeting QIDP status. Experience with Electronic Health Records (EHR) and web-based programs preferred; valid driver’s license and safe driving record in accordance with RACS policy are required.
