Behavioral Health Specialist
The
Department of Human Services (DHS) is seeking an experienced
Behavioral Health Specialist to serve as a Case Manager. for the Behavioral Healthcare Division and provide services to clients with mental illness and/or substance use disorders. This position will provide assigned clients with outreach, engagement, support, coordination, and direct case management services and will collaborate with multidisciplinary providers including psychiatrists, medical doctors, therapists, the community and other natural supports. Full range of mental health services to special populations such as clients who do not speak or understand English take place mainly in the community and client’s homes with some in office meetings.
Specific duties include:
- Conducting comprehensive clinical assessments of clients;
- Developing, implementing, and evaluating treatment plans based on diagnosis and functional needs of client;
- Conducting risk assessments, crisis stabilization services and making determinations regarding placement and level of care decisions;
- Monitoring progress toward service goals, and facilitating appropriate termination of treatment;
- Linking clients to various resources in the community or within the agency based on their needs, such as housing, healthcare, food security, benefits, employment, and social supports;
- Working with clients and providing encouragement of functional skills such as money management and other self-help tasks;
- Coordinating services with internal and external partners, families, and clients;
- Completing extensive medical records that are reviewed to ensure compliance with clinical, funding, and program licensing regulations/standards;
- Participating on inter or intra-departmental task forces to assist in short- and long-range planning for the delivery of services; and
- Performing case management and related administrative duties.
