Community Health Worker Behavioral Health Liaison #4329
RESPONSIBILITIES
The Liaison will be a resource to the local area Community Health Workers and Community Paramedics to help navigate behavioral health providers, as well as provide referrals to partner agencies to reduce emergency department visits, connect individuals to primary care and behavioral health providers, and improve population health; identifies high-risk individuals with chronic medical and mental health needs and directs individuals to medical and mental health services.
REQUIREMENTS
Minimum Requirements
M1: Bachelor’s degree in health or social services related field
M2: Valid driver’s license and safe driving record
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
ANNUAL SALARY RANGE
$42,865-$75,014
