Healthy Families Case Manager/Family Support Worker #2096
RESPONSIBILITIES
Provides intensive home based case management services to children at risk of serious emotional disturbance and
their families. Promotes positive mental health development, family functioning, and parent-child interaction.
Develops and implements individual service plans and completes required documentation. Assesses families to
determine appropriateness for services. Links families with community services.
REQUIREMENTS
Minimum Requirements
M1: Bachelor’s Degree in Human Services (Psychology, Social Work, Counseling, or other related field)
OR
Registered Nurse with a valid license to practice in Virginia with one year experience providing direct service to individuals
with mental illness, intellectual disabilities, or individuals receiving gerontology services or special education services (which
may include supervised internship, practicum, and field experience)
M2: Must have a valid driver’s license
M3: Work schedule first shift hours with some night and weekends
Preferred Requirements
P1: One year experience providing direct service to individuals with mental illness, intellectual disabilities, or individuals
receiving gerontology services or special education services (which may include supervised internship, practicum, and field
experience)
P2: Experience in case management
P3: Experience in service planning process
P4: Home visiting experience
P5: Experience in working with children five and under
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be
required upon request. Our Agency maintains a drug-free workplace.
ANNUAL SALARY RANGE
$34,937 – $55,027
