ID/DD SUPPORT COORDINATOR
Southside Community Services Board (SCSB) seeks qualified individual for its Intellectual Disability Division in South Hill, Virginia, to provide case management services for adult individuals with diagnosis of intellectual disability with goals to maintain stability and allow those served to function at the highest level possible. Duties include: admissions/intakes; development of individualized service plans (ISPs); coordinating community service delivery programs; and pre-admission and pre-discharge planning. Incumbent provides consultation/educational services to consumers/families in areas of crisis prevention, intervention and stabilization. Performs home visits and follow-up services, and maintains timely, accurate documentation as required by State and Federal regulatory agencies. Arranges transportation for individuals served as needed.
Bachelor’s degree in a human services field with at least one year prior work experience with the intellectually disabled required, along with knowledge of intellectual disability including treatment modalities, intervention techniques, and behavior management. Must maintain strict confidentiality, meet all criminal background and drug screening requirements, possess effective documentation/communication skills and valid driver’s license with safe driving record.
