Nurse Educator- LPN (Pos. #1082)
Colonial Behavioral Health is currently seeking a Nurse Educator-LPN to providing education on chronic diseases/management for individuals and staff.
The Nurse Educator is responsible for participating in delivery of patient education and disease management interventions and for performing health coaching for individuals, across multiple lines, for health improvement/management programs for chronic diseases. Position serves as a member of an integrated behavioral health and medical team providing care coordination to persons with serious mental illness and/or developmental disabilities with certain specific serious and chronic medical conditions.
Work Schedule: Monday – Friday; 8:30 am – 5:00 pm. Flexing of work schedule may be necessary to accommodate individual need.
Qualifications (EOE):High school diploma or equivalent required. Licensed Practical Nurse (LPN) granted by the State of Virginia Department of Health Professionals required. A minimum of one year nursing experience working with persons with serious mental illness and/or developmental disability preferred. Experience in a family practice setting with the psychiatric experience preferred. Valid VA Driver’s License and acceptable driving record required at time of employment and must be maintained thereafter.
