Case Manager (Young Adults)

Arlington County Government’s  Department of Human Services (DHS) is seeking an experienced, full-time Case Manager (Young Adults) to be a part of the Aspire First Episode Psychosis Team (FEP). This position will provide specialized Supported Employment and Supported Education Services (SEES) and case management services to individuals between the ages of 16-30 who are experiencing their first episode of psychosis. The position is in the Behavioral Healthcare Division of DHS and serves as a Case Manager on a team that includes a Peer Recovery Specialist, Psychiatrist, Therapist, and Team Lead. Case management services supporting employment and education will take place in both the office and in the community as the employee will be actively involved with the families of those served. The Case Manager (Young Adults) will provide assigned clients with outreach, engagement, support, coordination, and direct case management services while applying knowledge of trauma-informed care and their experience working with clients who have both mental health and substance use disorders. This position is also responsible for conducting assertive outreach and engagement strategies with clients as well as the assessment of suicidality and for delivering basic safety and crisis prevention planning.
Specific duties include:
  • Providing individual vocational and educational support services based on diagnosis and functional needs to gain or strengthen independent living skills;
  • Working collaboratively to effectively identify strengths and barriers toward vocational and educational goals;
  • Creating and offering job development and education in the community, as well as to staff at high schools and local universities on FEP to increase awareness and advocate for academic accommodations;
  • Performing risk assessments to determine functioning level and services needs;
  • Documenting in the Electronic Health Record (EHR) according to standards, all treatment needs, and services delivered; and
  • Striving to meet policy requirements for accurate and timely clinical documentation and productivity standards.
