Adult Case Manager (3059)
RESPONSIBILITIES
Provides assessment, linkage, and advocacy services to individuals with developmental disability; develops and implements person-centered individual service plans; completes required documentation; and complies with all regulations applicable to program services.
REQUIREMENTS
Minimum Requirements
M1: Bachelors Degree in a human services field
OR
Bachelors Degree in related field with at least 15 credit hours of coursework in human services
OR
A Registered Nurse with a valid Registered Nurse license to practice in Virginia
M2: Valid drivers license with a safe driving record
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
ANNUAL SALARY RANGE
$42,865 – $75,014
