Child and Adolescent Case Manager (3344)
RESPONSIBILITIES
Provides assessment, linkage, and advocacy services to individuals age two to eighteen with a documented diagnosis of a developmental disability and/or who are dually diagnosed with a developmental disability and a serious emotional disturbance or at risk of a serious emotional disturbance if under the age of seven. Acts as primary facilitator in the coordination and integration of services for assigned caseload. Develops and implement Individualized Service Plans (ISPs); completes required documentation and complies with all regulations applicable to program services. Applies generally prescribed standard practices over a wide range of procedures with some latitude for decision making.
REQUIREMENTS
Minimum Requirements
M1: Bachelors Degree in a human services field
OR
Bachelors Degree in related field with at least 15 credit hours of coursework in human services
OR
Registered Nurse with a valid Registered Nurse license to practice in Virginia
M2:Valid drivers license and safe driving record
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
ANNUAL SALARY RANGE
$42,865 – $75,014
