ICF Support Coordinator (3365)
RESPONSIBILITIES Under the supervision of the Director of Residential Services, the ICF Support Coordinator is responsible for ensuring that individual support services are in compliance with all federal, state, local and agency regulations; completion of functional assessments, writing skill building supports and monitoring each individual’s progress; staff training and completing monthly summaries; attending Interdisciplinary Team Meetings and the Specially Constituted Committee Meetings; develop and implement individual’s service plans; complete required documentation; comply with all regulations applicable to program services; and assures individuals are treated with dignity and respect.
REQUIREMENTS Minimum Requirements M1: Bachelor’s Degree in a Human Services field OR Registered Nurse with a valid RN license to practice in Virginia M2: At least one year clinical experience working with individuals with developmental disabilities M3: Must have a valid driver’s license and safe driving record Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. ANNUAL SALARY RANGE $40,711 – $71,245 5% Increase for Qualified Mental Health Professional Excellent Fringe Benefits
