This position is responsible for assessment, service planning, care coordination and ongoing monitoring to ensure appropriate utilization of services and continuity of care.
Typical tasks include:

- Consumer assessment, treatment planning and monitoring
- Community outreach to assist consumer in maintaining treatment compliance
- Documentation of all services in electronic medical record
- Quarterly reviews of consumer progress and of active service plans in compliance with Medicaid and DBHDS requirements
- Coordination of consumer services with services providers, community partners and familial supports on behalf of consumer
- Assist with discharge planning for consumers that have been hospitalized, including linkage to community resources

Vocational/Educational Requirement: Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field or other degree as approved by the Virginia Department of Health Professions. A master’s degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.
Experience: In addition to satisfying the vocational/educational standard, this class requires a minimum of two years of related, full-time equivalent experience.
Special Certifications and Licenses: Requires a valid driver’s license and a driving record that is in compliance with City Driving Standards.
Requires a Qualified Mental Health Professional (QMHP) or Qualified Intellectual Disability Professional (QIDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid and medication administration certifications within 3 months of hire, Certified Substance Abuse Counselor (CSAC) certification upon hire, Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire, Part C Early Intervention Certification within six months of hire, and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).
Special Requirements: Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.
