ACT Case Manager (CSAC) for our Assertive Community Team (ACT). ACT is a highly coordinated set of services offered by a group of medical, behavioral health, peer recovery support providers and rehabilitation professionals in the community who work together as a team to meet the complex needs of individuals living with severe and persistent mental illness.
Responsibilities
This position serves as a specialty case manager for the interdisciplinary ACT (Assertive Community Treatment) team that serves adults with severe and persistent mental illness in the Greater Reach CSB service delivery area. This position will function as the ACT team’s substance abuse specialist, providing recovery treatment, support, and coordination of services to individuals with identified substance use needs. This position carries out rehabilitation and support functions, based on the integrated Dual Disorder Treatment model, focusing on assessment and treatment, using principles of harm reduction, motivational interviewing, and CBT, under the direction of the team leader.
Required Education and Experience
- Bachelor’s degree in a human services or related field.
- Registered with the Virginia Board of Counseling as a QMHP, or be registered with the respective boards as a QMHP-T or QPPMH, under supervision.
- Registration with the Board of Counseling as a Certified Substance Abuse Counselor (CSAC).
- Two (2) years of experience providing substance use treatment to adults with co-occurring serious mental illness and substance abuse disorders in a mental health setting.
- Valid Driver’s License with a point balance of -6 or higher.
Preferred
- Three (3) years of experience working with dually diagnosed adult consumers in a mental health setting.
Required Knowledge, Skills and Abilities
- Must be well versed in substance use treatment modalities and stages of change.
- Must demonstrate strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
- Demonstrated competence and ability to establish supportive trusting relationships with people with severe and persistent mental illnesses and respect for individual rights and person preferences in treatment are essential.
- Ability to conduct required home visits are required, drive an agency vehicle to conduct agency business.
- Required to obtain and maintain CPR certification.
- Ability to make sound clinical judgments.
- Must have the ability to tolerate light lifting, bending, stooping, reaching, and climbing stairs.
- Ability to successfully manage a caseload of 10 or more consumers.
- ACT is an essential program, operating 365 days of the year. Applicant must be able to work some evenings (until 6:00pm), weekends, and holidays, based on program needs, and work with a rotating on call schedule.
- Position provides community-based co-occurring treatment that will require entering homes and/or community centers, medication delivery, medication education and monitoring activities, daily living assistance, transporting clients, family support and case management.
- Will be required to travel within the Greater Reach CSB catchment area.
Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection. Our Agency maintains a drug-free workplace.
Annual Salary Range
Grade S-C8
$57,965.00 – $78,978.00
