Phone call triage for medication refills, form requests, crisis intervention, etc. Medication prior authorizations. Respond to crisis situations which may include Emergency Services contact and participation in the TDO and/or ECO process. Care coordination through collaboration with a multidisciplinary team (registered nurses, prescribers, case managers, therapists, etc.) Provide documentation in the electronic health record system. Completion of primary care screenings (height, weight, auto-populated Body Mass index (BMI) and vital signs. Complete referrals, tardive dyskinesia screenings, as well as fall risk assessments. VOCATIONAL/EDUCATIONAL REQUIREMENT: Requires a diploma in practical nursing. EXPERIENCE REQUIREMENT: In addition to satisfying the vocational/education standards, this class requires a minimum of one year of related, full-time equivalent experience. SPECIAL CERTIFICATIONS AND LICENSES: Requires licensure as a Licensed Practical Nurse by the Commonwealth of Virginia or a reciprocal state (according to the Nurse Licensure Compact as prescribed by the National Council of State Boards of Nursing). Requires certification in CPR, Multi-media First Aid, and Handle With Care. Requires a valid driver’s license and a driving record that is in compliance with the City’s Driving Standards. SPECIAL REQUIREMENTS: Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies.
